Billing

The cash-pay rate for a standard session is:
$100 for an Intern-Level counselor
$125 - $145 for an Associate-Level counselor
$220 for Emily Brems, the founder of Living Well
Sliding scale and scholarship rates are available by request but are subject to approval and generally only granted for 1-3 months at a time.

We take cash and all major credit cards (including FSA/HSA cards).

We currently only accept OHP (Medicaid) through Open Card plans.
While we do not accept private insurance, most insurance companies will reimburse costs for mental health services when given a “Superbill” depending on your plan.
Before going this route we recommend asking your insurance provider these questions:
Does my plan include mental health benefits?
Do I have a deductible? If so, what is it and have I met it yet?
Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
Do I need written approval from my primary care physician in order for services to be covered?
Can I be reimbursed for mental health services by submitting a superbill?

A Superbill is a document which can be submitted to insurance providers for reimbursement.
Superbills include the following information:
Our practice location, contact information tax ID, and NPI number
Your provider’s NPI and license numbers
Your diagnosis code(s)
Service dates and CPT code(s)

All of our counselors offer both in-person and telehealth appointments with the flexibility to switch between the two as needed.
More advanced notice is appreciated when converting a telehealth appointment to in-person.

Standard sessions last 53-60 minutes.
Longer appointments are available, but they’re usually limited to cash-paying clients due to insurance providers rarely covering them.

If you are unable to attend a session, please make sure you cancel at least 24 hours in advance.
If a cancelation happens within 24 hours of your appointment you will be charged the full rate of the session unless otherwise arranged with your counselor.
